This is an interesting but uncontroversial post (!) These are modern medical miracles.

Jeff was my roommate in medical school and later trained at one of the top orthopedic programs in the USA. He was a better student than I was. As kids, we climbed, weight lifted, and flew hang gliders together. We are now old dogs, so we compared injuries, and I have had more. Our dialog will amuse you. Here are a few other things we discussed:
Jeff says that stem cell injections work poorly for arthritis.
Steroid injections and cartilage removals create joint destruction and lead to the big surgery sooner. Both are a bad idea.
The operation time for joint replacements is usually short—between an hour and two hours.
Anticoagulation is used for months after some joint replacements.
Some people can use joints that seem worn out, according to the X-rays. Since these surgeries are done for symptoms, you do not need the procedure if you have little pain. Jeff said that if you have no joint space, most people get better with an implant.
Checking the urine and the teeth for infection before joint replacement is prudent because infections are life-altering and can spread from sources like these. Dental “spiral CT scans” can be used to be sure patients do not have hidden abscesses of the mouth.
I studied this subject when considering what to do with my sore shoulders, and I thought my listeners would like more information. Another factoid: RFK, Jr. told me that he had all six of his major joints–his hips, shoulders, and both knees–replaced or revised. All are working well.
The general principle is that you never consider these surgeries unless you have significant pain and are taking painkillers to sleep at night. If your discomfort is tolerable, you do not want to endure the risk, hassles, and recovery of a joint replacement or other surgery for orthopedic problems. Even if your X-rays look terrible, sensible surgeons do not recommend reconstructions or joint replacements when symptoms are modest. Many situations will improve if you are patient and carefully rehabilitate yourself.
You can investigate your case by getting x-rays and seeing the orthopedist if you have chronic, moderate pain. Find out who has the most experience with the procedure you might need and see what they think.
The hip replacement procedure won an award as the safest and most effective surgery of the 20th century. This is not entirely true; cataract surgery is better. Hip replacements rarely have complications, but if they get infected, it is life-altering. On the other hand, cataract surgery has almost no problems in experienced hands and consistently improves vision. Some newer lenses can help you see near and far, like bifocals.
Although knees are done more commonly, shoulder replacements are the second most predictable total joint surgeries after hips. In experienced hands, however, shoulders have excellent results with proper rehabilitation. If your tissues and body are reasonably healthy before the surgery, your chances of a perfect outcome are high. I had little pain and took no opioid painkillers after mine.
My friends have all had great results and needed no pain drugs after their knee replacement recoveries. However, published reviews of this surgery show that up to 50% of patients still require painkillers following the surgery. These also get infected more often than hips or shoulders. The factors that predict excellent results are good health, bone quality, and the surgeon’s skill. Skill is directly related to the number of cases done—experience. Always ask your surgeon how many like yours he has performed.

I have osteoporosis. Can hormones help me? This is a significant health issue for elders—twenty percent of those over 65 breaks a hip. Many become nursing home residents; depending on the study, up to 58 percent are dead within a year. This dwarfs breast cancer as a public health problem. Although hip fractures occur mainly in women, they can also happen in men.
Hattie is 64 years old and works as a federal prosecutor. She is small, single, Chinese, and stressed. Her physician diagnosed her with “osteopenia” a decade ago. She treated Hattie in the office to a regimen of bisphosphonate injections and has followed her with dual-energy X-ray absorptiometry (DEXA) scans. But Hattie’s scan scores have gotten worse. When she had a hip replaced, the orthopedic surgeon told her that the bone was “crumbly.” Two years ago, Hattie began estrogen, selenium, DHEA, progesterone, vitamin K, testosterone pellets, and 10,000 IU of vitamin D. She recently started ipriflavone. Since beginning hormones, she can do jury trials and other high-intensity legal work like an attorney half her age. She was able to cut her Prozac dose in half. Her repeat DEXA showed that her bone density increased by 7 percent in two years. She is starting HGH and hopes to improve even more.
Physicians working with osteoporosis try first to eliminate causes such as steroids, smoking, and other drugs. After this, the pricey, FDA-approved bisphosphonate medications are usually tried. The industry promotes them for the prevention of bone thinning, but they are worse than nothing. They cause fractures, rotting jawbones, and even esophageal cancer. TheNNT.com summarizes the data, quoting a Cochrane meta-analysis. They say the drugs are ineffective for women without prior fractures and that they prevent bone breakage solely in the small group of women who have both osteoporosis on a scan plus a previously broken bone.
These facts make it hard to fathom why there are 80 million people using these drugs in the US. There is at least one good reason, however: our good doctors get about a 20 percent commission for dispensing medications like these in their offices. This is a terrific conflict of interest and would be criminal fee-splitting if physicians made a deal like this with another doctor instead of with a corporation. Another similar case is Lupron, the chemical castration agent used for prostate cancer. You and your insurance company pay $10,000 for each shot, and the doctors get their cut of the loot. Although the original studies claimed these drugs decrease bone breakage, this medication class increases fractures of the largest bone in the body, the femur. They also produce cases of irreversible jawbone rotting. Plaintiffs have filed thousands of lawsuits against Merck alleging their bisphosphonate, Fosamax, caused these disasters. The company paid $28 million to settle twelve hundred of them. These drugs also increase the risk of atrial fibrillation.
Most people hate this medication class because the drugs make them feel terrible. One website lists over 40 side effects. In contrast, hormone treatment makes most people feel wonderful along with preventing and reversing osteoporosis. DHEA, testosterone, vitamin D, and estrogen all have positive effects. HGH alone can improve bone density by eight percent a year.
Here are some non-hormonal treatments that are safe but more speculative. Selenium may help at a dose of one gram a day, increased after a month to two grams a day. Vitamin K2, one milligram per day, might also be valuable. Ipriflavone, 600 mg per day, is an approved prescription treatment for osteoporosis in Japan that is over-the-counter in the US. It may help if you already have the condition.
I had progressive pain for five years in my left more than my right shoulder. I took painkillers almost every night for years to sleep. I had some fantasies about stem cell injections, but after a friend had successful shoulder replacements, I decided to get a consultation. Fortunately, I found a surgeon who had done more than 1000 and had been trained by the top shoulder replacement surgeons in town (Dan Acevedo, MD, at Kaiser Panorama City). So I decided to proceed.
See my X-rays above. In recent years, orthopedists learned that long stems on the steel caps caused problems, so these were eliminated. The socket (glenoid fossa) is made of plastic and cemented in place with the same special glue used for the humeral head implant.
My surgeries were performed 4 1/2 months apart. Other surgeons recommend waiting six months to a year to allow complete recovery from the first shoulder. About 12 weeks following a shoulder replacement, the surrounding tissues become durable and can tolerate range of motion and strengthening exercises. Before that, only moderate stretching is recommended. I was nearly pain-free two weeks after each surgery.
These artificial shoulders last 15 to 30 years, about my expected lifespan. Replacing them is a hassle, but it can be done if they wear out for some reason. I plan to swim, lift weights, and do a little rock climbing. Wish me luck—at 69, I need it.
About the comments: I know that I am an acquired taste. The friends I cherish are the ones who tell me when I am wrong so I can sharpen my messages against their intellects. I had progressively worse nasty knee pains in my 40’s, but it was not from wear and tear, but rather from joint poisoning, caused by grain consumption. I had found that consuming, at least, 1 tsp Ginger and 2 tsp of Turmeric powder daily, with a pinch of Pepper, was far more help than any alleged “Joint Repair” supplement or painkiller at reducing joint pain.
The real problem is that Bad foods, like grains and too much plant toxins from other plant foods will cause damage, which most are clueless of, because of establishment deception about “plant foods” and joint pain. Dr. Darko Velcek has identified why joint pain occurs, toxins reduce cell hydration, including of the lining of the joints, thus joint pain. Plant “foods” are a common source of toxins, especially grains!
Any discomfort in any of my joints vanished after I had started seriously doing a low carbohydrate, high animal fat and flesh diet, with no grains; therefore, fixing your diet, then waiting enough time, should be done before any drug-based “medicine” or surgery is considered.
For fingers, some “joint pain” can be caused by too much finger clenching, which can be fixed by doing finger pushing exercises e.g. using elastomer/rubber finger exercise bands.

Thank you for the facts on Bisphosphonates and the many great options. The tests for osteoporosis are stupidly inaccurate and make it easy for clinicians to put fear in their clients. I have a friend who just broke her back from a fall and claims her RX for Bisphophonates saved her from other fractures. I knew she wouldnu2019t appreciate information on how these drugs work to slow bone repair. There are so many better alternatives to Bisphophonates and the list grows longer every day!

I’m more familiar with Forteo, it’s a 6-month shot, and comes with a “BLACK BOX BONE CANCER” warning. I’d look up Prolia. I used Vits/Mins to reverse OP back to Osteopenia in 6 years, 3 main spots, hips and one spinal area didn’t see as much change. Do more research on natural treatments.

The harm caused by metal implants is hidden. Most surgeons donu2019t seem to have any knowledge of type 4 allergy to metals or type 1. Pharmaceutical companies are working on metal free implants including implants which dissolve. Many who are killed by a bad reaction to an implant are told by drs they think a mystery virus has got to their heart then they die. In NZ they will refuse to consider harm caused by metal orthopaedic implants. Metal implants are also a cause of osteoporosis. I have osteoporosis myself. Type 4 allergy to nickel is also a big cause of osteoporosis in my own investigations. Many more women than men get osteoporosis. Ages ago, I was in a Cancer Cured group with Dr. Rick Linchitz. A tri-athlete, he most horribly suffered with metalllosis. subsequent to a hip replacement. How to chelate out all of that metal induced by the metal-on-metal hip replacement? Tragic, really.
I know a lady who had one of those dubious metal-on-metal hip replacements, and she had to have revision surgery. She was involved in a class action lawsuit and is awaiting her $100,000. settlement.

Nickel allergy is common but I’m not sure how vulnerable people respond to the high grade steel used for implants.
As with any antigen or foreign object there will be an Immune response. The metals in stainless steel cause a very strong immune response in those susceptible and so do those in titanium implants. Iu2019ve had to have two titanium alloy spine implants removed. If I didnu2019t they would have killed me. Iu2019m quite well known around the world on biological responses to metals with patients and also specialists. Simply because of the knowledge Iu2019ve gained from multiple implant allergies and metals in dental work and other sources over my life. And my exposure to specialists who are at the top of this field.

Thank you Dr. Yoho! Timely for me! I am looking at a probable knee replacement early next year. My knees were doing pretty well until last month when I stood up suddenly and felt a terrific searing pain in the right one which felt like something tore. Xray showed severe osteoarthritis – which I knew. The orthopedic doctor there said the only help was a replacement. I did not get a comfortable feeling about his competence. I did find another possible doctor a couple of hours away. I know the bones are not in great shape, so I appreciate your advice on supplements and hormones. Right now I need a walker to keep the knee from getting too much usage causing soreness. The pain has made it clear I need to do something different from rest. Any further advice on what to look for in a doc would be appreciated! Does this orthopedic doc give further advice as well? Thank you!

When I was 34, 32 years ago I had a giant cell tumor in my hip. Ignored the pain but years later they discovered the tumor by accident. 90% of my hip was gone and I was told not to put any weight on it until the operaton. I got lucky and found an apparently well respected Beverly Hills surgeon Mark Stern, an old guy with coke rimed glasses. He replace the tumor with bone chips against the advice of a young surgeon who wanted to use cement. Dr. Stern said one day I’ll get arthritis. Before long I was running with no problem.
Maybe 10 years ago I started waking up with pain in my hip and I figured the old doc was right. Then I heard about nightshade plants making arthritis worse if you have it but not causing it outright.
So I gave up black pepper and cayenne pepper and the pain went away. Introduced them again and the pain returned.
What is bad for me but be fine for you. Here’s a list food to consider testing to see if it makes your if arthritis worse.
Tomatoes, potatoes (sweet potatoes and yams are not nightshade) peppers (red, green, yellow, any variety) paprika, goji Berries, cayenne, chili powder, eggplants and okra gooseberries, tobacco sorrel, ground cherries

Good interview. Thanks. I had hip and knee osteoarthritis 10 years ago, doc decided to do the hip first. Painless recovery, joint still working great 10 years later with very active use. The most amazing part was replacing the hip also healed the knee pain. Apparently the hip pain made me walk wobbly which stressed the knee. Now I climb mountains with no meds.
I’m grateful to the doc, and for the technology that made it so easy. I did clean up my diet and shed pounds (BMI 30 to 20) so I’ve tried to be a good patient.

Thanks. Diagnostics found a torn meniscus and he repaired that first, and the photos showed some wear in the joint. Helped the knee pain, but I still couldn’t sleep with my leg straight, used a bolster to raise my knee. Surgeon said we should do the hip next, which was pretty ugly, and would probably need to do the knee later. Hip healed well and knee pain disappeared. Losing the weight probably helped. He did an excellent job in both surgeries and apparently had good instincts about the sequence. Ten years later, both joints still doing fine but I keep track of the surgeon in case I need him again.
Both joints were right leg, which I used to kick start big dirt bikes for many years. I assumed that caused it, doc said, no, I’m just old. Left leg still fine. My evidence was circumstantial, his was more objective. But I don’t ride much any more, for other reasons.

Is that one gram and two grams a day a typo? The RDA for selenium for adults is 55 micro-grams per day and tolerable upper intake level for adults is 400 micro-grams per day according to:

As a farmer I supplement my livestock with selenium as do most other people who own livestock in the northwest. Consequently, any food grown in the northwest is also low in selenium. Just like with taking D3, the RDA is supposed to cover most people, but the CDC and refuses to recommend supplementing at a higher level, even though people who do are much less likely to be hospitalized if they end up with covid.

“cataract surgery is better” ?
Cataract can be treated with eye drops containing N-acetylcarnosine (NAC) but the product can be difficult to obtain. Patients are not being informed about the option, just gaslighted into surgery that often destroys the eye’s ability to focus. Two major brands are Ethos Bright Eyes and CAN-C but there are also formulations for dogs that are more widely available. Sometimes our pets are treated better than humans.
There is also EV06 eye drops that effectively treat presbyopia but development has stalled since Novartis took over Encore Vision.
That may only treat the symptoms, not the cause, Glucose and other toxins! Dogs may only get cataracts because of the cruel idiocy called dog biscuits and any other harmful plant matter in manufactured alleged “dog food”; dogs are carnivores!
Cataracts are most probably caused by AGE (Advanced Glycation End-product) damage from excessive blood Glucose. Consuming much sugar and/or starch is a very bad idea because elevated blood sugar always causes damage, including via the mitochondria, and this damage can be cumulative, including via AGE formation!
Man is clearly a top tier carnivore, only OK for limited eating of seasonal fruit (Fructose and no Glucose), despite common plant farming and Diary farming originated idiocy/propaganda.

From my hub’s viewpoint, it’s a mixed bag, Years of shots for the knee. His Ortho was not well welcomed in the ‘shark pool’ he liked partial knees, rather than full if it was not needed. The outcome depended on you not falling during the healing period. Shoulders were too painful to lift, #1 was a total clean out, #2 was a ball replacement, and BOTH were Failures. The Ortho himself has a massive rotator injury and uses Stem Cells. it is not always available to the average income patient or retired person on Medicare/Tricare Life.
Since Medicare mandated full knees, he only does private insurance, or if it will cover the cost of stem cells. His practice is very busy, with no partner.
The big Ortho Center has many types but is a group-think institute. Steroids, are a huge part, even if you aren’t supposed to take them. I’m hearing of many more ‘self-contained’ practices, Gastro, Kidney, and Vision are just three more, that are separate from in-hospital surgeries. Some are good, others are to be avoided. My first Meniere’s attack was a Hearing Self contained one, was a nightmare., And it does no good to file complaints, as they are “Stand Alone’s”.
